Advanced Practice Provider - Behavioral Health
About The Position
Sentara Health is seeking a Nurse Practitioner or Physician Assistant to join our Behavioral Health team in Harrisonburg, Virginia, providing care for adolescents in a Partial Hospitalization Program (PHP). This role offers the opportunity to support adolescents who require a higher level of support than standard outpatient care but do not meet criteria for inpatient hospitalization. Patients attend the program Monday through Friday from 9a-3p. The APP will provide care to participants actively enrolled in the program, as well as to recently discharged patients who are awaiting transition to community-based outpatient providers.

Responsibilities
- Conduct psychiatric evaluations, treatment planning, and ongoing medication monitoring.
- Collaborate with a multidisciplinary team delivering group and individual therapy services.
- Maintain an average daily schedule of approximately 10 patient visits, including both onsite and follow-up care.
- Provide transitional follow-up care for patients after discharge, typically for 3 to 6 months, until they establish with a community outpatient provider.
