About The Position

Join our San Jose, California team! This is a mission-driven advanced practice role providing comprehensive, patient-centered care within an integrated community health setting focused on whole-person wellness and collaborative care. We are seeking a compassionate and experienced Nurse Practitioner to provide high-quality medical care to patients across multiple clinic locations in San Jose. Working under the supervision of a physician and as part of an integrated care team, this role focuses on delivering comprehensive primary and preventive healthcare services to diverse patient populations. The ideal candidate is collaborative, culturally responsive, and passionate about serving the community. This individual will assess, diagnose, and treat patients while promoting wellness, preventive care, and chronic disease management in a fast-paced outpatient setting. Key responsibilities include conducting physical assessments, reviewing medical histories, prescribing medications, developing treatment plans, educating patients on disease prevention and medication compliance, coordinating referrals to specialists, and maintaining accurate clinical documentation in compliance with regulatory standards. The Nurse Practitioner will also collaborate closely with behavioral health and wellness teams, participate in quality improvement initiatives, support clinical audits and reporting, attend team meetings, and provide coverage for other clinicians as needed. This role also includes participation in an after-hours on-call rotation and requires the ability to work effectively with patients and colleagues from diverse cultural and socioeconomic backgrounds.

Requirements

  • Master's Degree or higher in Nursing
  • Current California Nurse Practitioner license required
  • Current DEA registration required
  • Minimum of 2 years of clinical experience
