About The Position

The ASA Case Manager is responsible for providing person-centered care coordination for individuals with developmental and intellectual disabilities to promote a high quality of life. Ariel Clinical Services has been in business for over 30 years and has grown significantly since its early days of serving individuals with intellectual and developmental disabilities (IDD). Today, we offer a broad range of services including residential programs, vocational support, foster care and adoption, therapy, and much more. Requirements

  • High school diploma or GED.
  • A combination of experience with IDD population, case management, and/or formal education totaling 4 years; OR a bachelor’s degree in any field.
  • The ability to communicate respectfully and effectively in both individual and group settings.
  • Ability to be flexible, and able to change direction rapidly with a positive attitude.
  • Ability to work independently, prioritize workload, complete a wide variety of tasks in a timely and accurate fashion and direct others, as needed.
  • Basic computer knowledge.
  • Exercise independent, ethical judgment within the scope and boundaries of the department, the Agency’s Standard Operating Procedures, and Federal, State and Local Rules and Regulations.
  • Ability to organize and present case material in a straightforward and concise written manner.
  • Ability to provide reliable, insured transportation for self and others through the community served.
  • Ability to be a reliable and supportive team member.
  • Ability to always represent Ariel in a professional and respectful manner.
  • Knowledge of community resources (social, public agency, and information) to assist clients in accessing these resources.
  • Ability to demonstrate consistent and reliable attendance as scheduled and requested.
  • Valid Regular Driver’s License, with acceptable driving record.
  • CPR, First Aid – before contact with clients.
  • Safety Care – within 60 days of hire.
  • Informed Supervision –deviant sexual behaviors training - within 60 days of hire, may not be left unsupervised with clients until this training is completed.
  • QMAP – (Medical Administration Certification) within 60 days of hire, may not provide medication to clients without this certification.
  • Client Specific Training of individuals on caseload.

Nice To Haves

  • 1 year mental health experience.
  • 1 year experience with evidence-based behavioral support.
  • Bachelor’s degree in social or behavior sciences.

Responsibilities

  • Establish, build, and maintain rapport with individuals, independent contractors, and other members of the multidisciplinary team in order that optimal information gathering, and collective decision making may occur. These duties must be carried out in a manner that respects the rights and dignity of individuals, is driven by individual choice and promotes community inclusion.
  • Provide case management for individuals. Work closely with, and act as the liaison between collaborating agencies/multi-disciplinary team.
  • Develop, monitor, and document mandated services and programming, and ensure individual satisfaction. Ensure individual files are audit ready and in compliance with Volume 8 rules and regulations.
  • Understand and participate in all relevant assessments to ensure appropriate level of care for individuals.
  • Facilitate home visits and meetings to assess the needs and progress of individuals and intervene as appropriate. Provide medication as needed.
  • In a timely manner, effectively and appropriately communicate potential risks and/or threats to an individual or program to Ariel supervisors and collaborating agencies.
  • Participate in mandatory rotation for “on call” to support our individuals.
  • Participate in ongoing program evaluation to ensure program effectiveness.
  • Must work closely with other Ariel Programs and offices.
  • Performs other duties as assigned.
