About The Position

The Adult Palliative Care Counselor provides counseling and critical psycho-social support to palliative care patients through the Palliative Care Service consult team. This role is responsible for providing psychological services to patients and their families affected by chronic and/or life-limiting conditions. The counselor will conduct appropriate assessments of patient and family mental health needs and provide consultation to healthcare professionals regarding the impact of psycho-social factors on patients' illnesses.

Requirements

  • Master’s degree
  • 3 years’ related work experience in social work or counseling.
  • IDFPR (Illinois Department of Financial and Professional Regulation) issued Social Worker License OR IDFPR issued Clinical Soc Worker License OR IDFPR issued Prof Counselor License OR IDFPR issued Clinical Counselor License
