The Adult Nurse Practitioner assesses and manages a defined adult patient panel, providing new and established patients with a working diagnosis, treatment plan, and follow-up. The role focuses on managing chronic conditions—including diabetes, hypertension, heart failure, and COPD—through treatment adjustments and measurable progress toward agreed targets. The clinician identifies high-risk patients, conducts timely outreach or visits, and works to reduce avoidable emergency and hospital use. The practitioner orders and interprets diagnostics, prescribes within scope, and documents care to support continuity and accurate coding. They coordinate with physicians, nurses, specialists, and other care-team members on complex cases, while advancing preventive screening, patient education, adherence, and self-management. The role also tracks access and quality measures, including same-week availability for urgent needs, chart closure, and patient experience. The practitioner works alongside collaborating physicians, nurses, and medical assistants, using an EHR, e-prescribing, and on-site or coordinated diagnostics. Protocols, formulary guidance, peer consultation, structured onboarding, dedicated exam capacity, and support staff are provided. The position reports to the medical director or practice lead.
