About The Position

We are seeking a dedicated and compassionate Mental Health Case Manager to provide essential support to individuals in need of mental health services. This role is crucial in assessing and coordinating care for consumers, ensuring they receive the appropriate services in a timely and organized manner. Requirements

  • High school diploma with 12 semester credit hours in social sciences and two years of human services experience (including one year of direct mental health contact), OR High school diploma and five years of direct mental health care experience.
  • Act 33/34 clearances.
  • Access to a vehicle for work-related travel.

Nice To Haves

  • Prior case management experience preferred.
  • Bachelor’s Degree in sociology, social welfare, psychology, or related field.

Responsibilities

  • Adheres to the Code of Ethics with consumers, co-workers, providers, and other stakeholders.
  • Interview prospective mental health consumers to obtain psychosocial data and determine eligibility and service needs.
  • Arrange for necessary professional evaluations and explain available services through the agency and County MH/ID Program.
  • Assess emergency and “immediate need” situations and make appropriate referrals, including facilitating voluntary and involuntary commitments.
  • Ensure completion of initial and annual financial liability determinations.
  • Utilize assessments to determine appropriate levels of case management services and complete referrals and authorizations.
  • Maintain compliance with intake protocols and procedures.
  • Provide bi-weekly phone contact for individuals pending TCM services.
  • Coordinate and facilitate access to mental health services and ensure continuity of care.
  • Facilitate interagency and ISPT meetings.
  • Monitor participation and progress through regular contact.
  • Complete required assessments, documentation, and face-to-face visits as scheduled.
  • Maintain ongoing relationships with consumers, families, and service providers.
  • Participate in discharge planning and advocate for consumer needs.
  • Respond to crisis situations and provide appropriate referrals.
  • Prepare reports, maintain documentation, and participate in meetings and trainings.
  • Support agency mission, vision, and recovery-focused philosophy.
  • Complete additional assignments as directed by the Supervisor.
