About The Position

This position will be for an Adult Day Care Program and then transition to the St. Luke Facility upon completion of the initial assignment. The primary role involves assisting residents with all levels of activities of daily living (ADLs) and meeting any other resident needs in accordance with facility policies and procedures.

Requirements

  • Completion of a 100-Hour Nurse Aide Training Program.
  • Completion of New York State Residential Health Care Facility Nurse Aide clinical skills and written exam and PROOF of certification.
  • If certified in another state, must have obtained New York State certification via reciprocity.
  • Sense of responsibility to job/residents, moral integrity, and understanding.
  • Ability to work cooperatively with others as part of a team effort.
  • Good mental and physical health required.
  • Sincere desire to work with the aged.
  • Pleasant, cheerful, tactful, and cautious with residents, relatives, visitors, and other staff members.
  • Flexibility and ability to remain calm in emergencies.
  • Treat resident information confidentially.
  • Knowledge of resident's rights.
  • Existing employees must have a satisfactory work record in their prior positions.

Responsibilities

  • Assist residents with personal care including oral/denture hygiene, bathing, foot care, nail care, hair care, shampooing, shaving, grooming, and dressing.
  • Transfer, ambulate, and transport residents.
  • Provide active/passive ROM (range of motion) with daily care.
  • Assist residents with toileting needs, including peri care and catheter care.
  • Maintain resident rooms in an orderly fashion.
  • Serve meals, provide nourishments and fluids, and feed or assist residents as needed.
  • Check vital signs (temperature, pulse, respiration, blood pressure) and alert the nurse immediately of any unusual findings.
  • Clean and maintain equipment and utility rooms as assigned.
  • Obtain and record resident weights in CareAssist.
  • Complete documentation, including ADL monitoring, special device records, I&O, toileting schedule, intake studies, and BM records, in a timely manner in the Matrix Care system.
  • Take all breaks off the floor and check with the Charge Nurse before leaving the floor for any reason.
  • Observe resident behavior and report any changes or unusual findings to the Charge Nurse.
  • Assist residents in preparing for doctor's visits, religious services, rehabilitation services, diagnostic services, and social events.
  • Provide prompt and efficient assistance to residents and answer call bells promptly.
  • Keep incontinent residents clean and dry, changing clothing and/or linen as necessary.
  • Provide peri care to residents as required.
  • Change bed linen and make beds in an appropriate and timely manner.
  • Turn and change the position of residents.
  • Lift residents as needed.
  • Attend 12 hours of in-services per year (6 per 6 months) and attend nursing meetings.
  • May accompany residents on outings and/or medical appointments.
  • Perform other related duties as may become necessary or as directed by the Supervisor, Department Head, and/or Administrator.
