About The Position

Under the supervision of the TCOOMMI Program Director. Receives and processes all TDCJ and local parole referrals from the Parole Departments in the following counties: Parker, Erath, Hood, and Palo Pinto. Performs assessments and assists in gathering information for recovery planning for all parole referrals. Provides case management, skills training, psychosocial rehabilitative training, and recovery plan reviews, as well as, extensive linking within the community. Ensures that all new clients’ complete benefit eligibility, medical, and medication services within set TCOOMMI guidelines. Will maintain a minimum caseload of 20, maximum of 25 Adult Case Management clients and will ensure each of them has a minimum of 3.5 hours of direct service hours each month. Coordinates monthly contact with parole officers regarding clients, IDT’s/joint staffing’s, sanctions, or motions to revoke in order to explore all possible alternatives to incarceration. Compiles and completes all data related to parole Adult Case Management. Participates in after hour’s crisis rotation. Extensive in region travel required.

Requirements

  • Bachelor’s degree in a human or social services from an accredited college or university.
  • Valid Texas driver’s license.
  • Acceptable driving record.
  • Personal automobile liability insurance as required by the state of Texas.
