About The Position

Under the direction of the Unit Supervisor, functions as a team member providing professional services in accordance with the person-centered plans of person served, and coordinate the provision of supports and services for adults with serious and persistent mental illness and/or developmental disabilities who need assistance in accessing, coordinating and monitoring resources and services. Assists the individual and their support network to identify, select, obtain and coordinate both paid and unpaid or natural supports to enhance the person's independence, integration, and productivity consistent with the person's capabilities and preferences as outlined in their Individualized Plan of Services (IPOS); performs related work as required.

Requirements

• Bachelor’s degree in Human Services or a related field (e.g., Child Development/Family Relations, Occupational Therapy, Community Mental Health, Chemical Dependence, Physical Therapy, Counseling, Criminal Justice, Recreational Therapy, Rehabilitation Counseling, Social Work, Psychology, Education, or Sociology).
• Valid Michigan Driver’s License.
• Current automobile insurance.
• Use of a personal vehicle during work hours.
• Willingness to transport persons served using personal or agency vehicles.

Nice To Haves

• LBSW or LMSW
• One (1) year of clinical experience with adults with severe mental illness or developmental disabilities.
• Veterans must submit Form DD214 to determine eligibility for Veterans’ Preference.

Responsibilities

• Provide professional services in accordance with person-centered plans.
• Coordinate the provision of supports and services for adults with serious and persistent mental illness and/or developmental disabilities.
• Assist individuals and their support networks in accessing, coordinating, and monitoring resources and services.
• Help individuals identify, select, obtain, and coordinate paid and unpaid or natural supports.
• Enhance the person's independence, integration, and productivity consistent with their capabilities and preferences as outlined in their Individualized Plan of Services (IPOS).
