About The Position

This role involves providing case management services to adolescents, focusing on their academic and social-emotional well-being. The Case Manager will coordinate with clinical and medical teams, communicate with schools regarding Individualized Education Programs (IEPs) and online schooling, and actively monitor students' academic progress. Responsibilities include assisting with homework, providing feedback, building connections with diverse students, and maintaining accurate records. The position requires a Bachelor's degree in a behavioral science or related field and at least one year of full-time experience with children in a social service setting. The role is full-time, Monday through Friday, with a pay rate starting at $43,680.00 per year. The work location is in person in Meridian, Idaho.

Requirements

  • Bachelor’s Degree in a behavioral science or related field
  • At least 1 year of full-time work experience with children in a social service setting
  • Must be minimum 21 years of age
  • Must pass background check
  • Must pass drug screening
  • Must be able to climb up and down stairs, hills, bend and lift 25+lbs

Nice To Haves

  • Enhanced background clearance with the ID Dept of Health & Welfare (preferred- will be required to obtain)
  • CPR/First Aid certification (preferred- will be required to obtain)
  • Current TB test (will be required to obtain)

  • Communicate with the school regarding IEP's
  • Be present during school work time, monitoring electronic use, and ensuring school work is being done
  • Reviewing classroom or curricula topics and assignments
  • Providing students positive and constructive feedback
  • Building meaningful connections with students from diverse backgrounds
  • Assisting students with homework, projects, test preparation, papers, research and other academic tasks
  • Recognizing different learning styles and student preferences
  • Collaborate with the treatment team
  • Maintain records and related documentation. Produces reports, correspondence and performs case management duties
  • Other tasks as assigned
