About The Position
A Case Facilitator I - Child and Adolescent operates under supervision of the assigned Area Director or designated QBHP/LP. This role is a member of a multidisciplinary treatment team, with primary job duties including providing case management and activities of daily living skills to child and adolescent consumers.
Requirements
- High School diploma or equivalent
- Must meet OBHP requirements
- Valid state issued drivers license
Nice To Haves
- Two years’ experience in mental health or community services preferred
Responsibilities
- Links, monitors, and refers clients to appropriate community resources
- Coordinates care with appropriate internal and external parties
- Ensures appropriate behavioral health services are provided
