About The Position

Select Home Health & Hospice is seeking a skilled and compassionate Registered Nurse – Admissions to complete patient admission visits across the Indianapolis area. This floating position supports both home health and hospice services, providing thorough initial assessments, individualized plans of care, and a smooth transition into services. The role includes in-home nursing care and additional patient visits as needed based on agency needs.

Select Home Health & Hospice is a clinician-led and operated skilled home health care agency serving central Indiana. It provides nursing and therapy services intended to support patients’ health, independence, and quality of life in their homes.

Requirements

• Graduate of an accredited school of professional nursing.
• Current Registered Nurse license for the practicing state of Indiana.
• Minimum of 1 year of nursing experience in a hospital, facility, or home health setting.
• Valid driver’s license, reliable auto insurance, and personal transportation.
• Current up-to-date TB test.

Responsibilities

• Complete admission visits and initial assessments for home health and hospice patients.
• Perform patient assessments and ongoing evaluations to monitor needs and changes in condition.
• Develop and implement individualized plans of care in collaboration with physicians and the clinical team.
• Complete required clinical documentation, including OASIS assessments for eligible home health patients.
• Provide in-home nursing care and additional patient visits as needed based on agency needs.
• Educate patients and caregivers about medications, diagnoses, treatment plans, and in-home care.
• Coordinate patient services and communicate changes in patient condition to physicians and the appropriate care team.
• Float throughout the Indianapolis area to provide admission coverage and additional nursing support as needed.
• Maintain compliance with applicable regulations, clinical standards, and agency policies.
