About The Position
Signature Healthcare at Home - Home Health is seeking a compassionate and experienced Registered Nurse (RN) to join our team as an Admissions Nurse serving the Beaverton/Hillsboro and Forest Grove area. This position plays a critical role in supporting patients and families by providing timely admissions, education, and coordination of care. The Admissions RN is responsible for responding to referrals, completing admission assessments, developing initial plans of care, and ensuring patients and families receive the education and support needed upon entry into home health services. This position works closely with physicians, referral partners, home health leadership, and the interdisciplinary team to create a seamless admission experience. Requirements
- Current Oregon Registered Nurse (RN) license in good standing
- Minimum one (1) year of nursing experience
- Valid driver's license, reliable transportation, and proof of auto insurance
- Strong assessment, communication, and organizational skills
- Ability to work independently and efficiently in the field
Nice To Haves

- Admission or intake experience
- Knowledge of Medicare hospice regulations
Responsibilities
- Respond to home health referrals and admission requests.
- Complete comprehensive home health admission assessments in patients' homes, assisted living communities, skilled nursing facilities, or other care settings
- Evaluate patient eligibility for home health services in accordance with Medicare and regulatory guidelines
- Coordinate admission paperwork, consents, and required clinical documentation
- Develop and initiate individualized plans of care
- Provide education regarding services, medications, symptom management, and available resources
- Collaborate with physicians, nurses and other interdisciplinary team members
- Communicate effectively with referral sources, hospitals, and community partners
- Ensure timely and accurate completion of documentation within the electronic medical record
- Support patients and families with compassion, dignity, and professionalism.
