About The Position

The Home Health Admit RN provides clinical nursing care to patients in their homes, combining Start of Care (SOC), wound care, and infusion therapy responsibilities. The nurse performs initial assessments, develops personalized care plans, coordinates interdisciplinary care, and educates patients and caregivers. This is a part-time position working Saturdays and Sundays.

Requirements

• Current Registered Nurse (RN) license in the State of Arizona.
• Strong clinical skills and a passion for patient-centered care.
• Expert organizational and time-management skills.
• Current CPR certification.
• Valid Arizona driver's license, proof of auto insurance, and reliable transportation.

Nice To Haves

• At least one year of home health experience is preferred.
• Wound care and/or infusion experience is preferred.

Responsibilities

• Perform Start of Care (SOC) visits, including initial physical assessments, medication reconciliation, and home safety evaluations.
• Create personalized care plans to improve patient outcomes and support patient independence.
• Perform wound assessments and provide wound care treatments when necessary.
• Administer and monitor IV/infusion therapies according to physician orders when required.
• Complete accurate and timely clinical OASIS documentation.
• Coordinate care with physicians, therapists, and the interdisciplinary team as a case manager.
• Educate and empower patients and caregivers.
