About The Position
Signature Healthcare at Home is seeking a compassionate, experienced Registered Nurse to serve as an Admissions Nurse in the Beaverton/Hillsboro and Forest Grove area. The role supports patients and families through timely home health admissions, education, and care coordination. The Admissions RN responds to referrals, completes admission assessments, develops initial plans of care, and works with physicians, referral partners, home health leadership, and the interdisciplinary team to provide a coordinated admission experience. The position involves working autonomously in a patient-focused environment and completing assessments in patients’ homes and other care settings.
Requirements
- Current Oregon Registered Nurse (RN) license in good standing.
- Minimum one year of nursing experience.
- Valid driver’s license, reliable transportation, and proof of auto insurance.
- Strong assessment, communication, and organizational skills.
- Ability to work independently and efficiently in the field.
Nice To Haves
- Hospice experience.
- Home health experience.
- Admission or intake experience.
- Knowledge of Medicare hospice regulations.
Responsibilities
- Respond to home health referrals and admission requests.
- Complete comprehensive home health admission assessments in patients’ homes, assisted living communities, skilled nursing facilities, or other care settings.
- Evaluate patient eligibility for home health services in accordance with Medicare and regulatory guidelines.
- Coordinate admission paperwork, consents, and required clinical documentation.
- Develop and initiate individualized plans of care.
- Educate patients and families about services, medications, symptom management, and available resources.
- Collaborate with physicians, nurses, and other interdisciplinary team members.
- Communicate with referral sources, hospitals, and community partners.
- Complete documentation accurately and promptly in the electronic medical record.
- Support patients and families with compassion, dignity, and professionalism.
