Careview Transitional Care of Weston is seeking a motivated and relationship-focused Admissions Director to lead census development and referral coordination for a skilled nursing facility. This position plays a critical role in managing facility admissions, building referral partnerships, and ensuring a seamless transition process for residents and families. The ideal candidate is highly organized, customer-service oriented, and experienced in healthcare admissions, case management, marketing, or post-acute care coordination.

Nice To Haves
- Previous experience in skilled nursing, healthcare admissions, case management, marketing, or related healthcare operations preferred
- Knowledge of Medicare, managed care, and post-acute referral processes preferred
Responsibilities
- Coordinate and manage all incoming admissions and referral activity
- Communicate with hospitals, case managers, discharge planners, and healthcare partners
- Review referral documentation and collaborate with clinical leadership regarding admission appropriateness
- Facilitate smooth resident transitions into the facility
- Maintain accurate admissions records, census tracking, and reporting
- Conduct tours for prospective residents and families
- Support facility census growth initiatives and relationship-building efforts within the community
- Work closely with clinical and operational leadership to support customer satisfaction and retention
- Ensure admissions processes remain compliant with company policies and healthcare regulations
- Participate in marketing, outreach, and networking activities as needed
