Requirements

Excellent communication, interpersonal, and organizational skills.
Strong computer proficiency, including Microsoft Office and electronic medical records.
Ability to multitask and work effectively in a fast-paced environment.
Professional, compassionate, and team-oriented attitude.

Nice To Haves

Previous admissions, healthcare, or customer service experience preferred.
Skilled Nursing Facility (SNF) or post-acute care experience is highly preferred.
Knowledge of Medicare, Medi-Cal, managed care, and insurance verification is a plus.

Responsibilities

Coordinate and manage the resident admission process from referral through admission.
Communicate with hospitals, case managers, physicians, families, and referral sources regarding admissions.
Verify insurance coverage and gather required documentation for incoming residents.
Conduct facility tours for prospective residents and families.
Maintain accurate admission records and ensure all paperwork is completed in compliance with state and federal regulations.
Collaborate with nursing, social services, therapy, and interdisciplinary teams to facilitate seamless resident transitions.
Track referrals, admissions, and census data.
Provide exceptional customer service while representing Medical Hill Healthcare in a professional manner.
Assist with marketing and community outreach efforts as needed.
