About The Position

Napa Post Acute is seeking a motivated, organized, and proactive Admissions Assistant to join its fast-paced, evolving Admissions Department. The role requires managing multiple priorities independently, anticipating challenges, and finding creative solutions while helping residents and families navigate the admissions process.

The position is full-time, starts at 10:00 AM, and requires weekend availability. The consistent schedule is to be determined. Pay is $19.00–$26.00 per hour. The position is located in Napa, California.

• Previous skilled nursing or healthcare admissions experience.
• Experience with PCC and hospital referral platforms.

Responsibilities

• Monitor and review incoming referrals, ensuring timely and accurate responses.
• Make appropriate admission decisions using established facility guidelines and recognize when escalation is necessary.
• Maintain consistent communication with the Admissions Director.
• Process incoming admissions, including PointClickCare (PCC) entry, room coordination, and required documentation.
• Complete admission packets and maintain timely, accurate documentation.
• Anticipate admission barriers and proactively identify solutions to maximize appropriate placement opportunities.
• Coordinate with nursing, housekeeping, and other departments to ensure smooth admissions.
• Conduct facility tours when the Admissions Director is unavailable.
• Independently balance referrals, admissions, and packet completion throughout the day.
