About The Position

VNA Hospice NWI is seeking a Full-time Admission RN to perform consultation and admission visits. The role involves assessing patient needs, completing admission documentation, and understanding the Hospice program and benefits. The RN will obtain orders for and procure immediate medications and needed DME/supplies, perform physical assessments of patients' condition, safety, knowledge, pain, and symptoms, and initiate a Plan of Care. Responsibilities include instructing on disease understanding, progression, and immediate interventions, and documenting assessments, current medications, and initiating appropriate care plans. The coverage area includes Porter, La Porte, Starke, Jasper, Newton, and Lake counties. Mileage is paid at the current IRS rate if a fleet car is not available.

Requirements

  • Unrestricted RN license in the State of Indiana.
  • Diploma must be from an approved school of professional nursing.
  • Must maintain current CPR certification.
  • Must hold and maintain a valid driver’s license, and automobile insurance.

Nice To Haves

  • Minimum of one year of medical/surgical experience in a hospital setting, nursing home, ECF, or hospice care preferred.
  • Previous hospice experience and/or demonstrates understanding and support of the hospice philosophy and concept preferred.
  • Must be sensitive to issues of loss and grief.
  • Excellent communication and organizational skills.
  • Nursing: 1 year (Preferred)
  • Acute Care: 1 year (Preferred)
  • BLS Certification (Preferred)
