About The Position

VNA Hospice NWI is seeking a full-time Admission RN to perform consultation and admission visits to assess patient needs. This role involves completing admission assessment and agreement documentation, understanding the Hospice program and benefits, obtaining orders for and procuring immediate medications and needed DME/supplies, performing physical assessments of patients' condition, safety, knowledge, pain, and symptoms, initiating a Plan of Care, instructing on disease understanding, progression, and immediate interventions, and documenting assessments, current medications, and initiating appropriate care plans. The position offers a $10,000 bonus based on experience and FTE status, mileage reimbursement if a fleet car is not available, and coverage for Porter, La Porte, Starke, Jasper, Newton, and Lake areas. The schedule is Monday through Friday, 8:00 am to 4:30 pm, with a minimum on-call rotation.

Requirements

  • Unrestricted RN license in the State of Indiana
  • Diploma must be from an approved school of professional nursing
  • Must maintain current CPR certification
  • Must hold and maintain a valid driver’s license, and automobile insurance
  • RN (Required)

Nice To Haves

  • Minimum of one year of medical/surgical experience in a hospital setting, nursing home, ECF, or hospice care preferred
  • Previous hospice experience and/or demonstrates understanding and support of the hospice philosophy and concept preferred
  • Must be sensitive to issues of loss and grief
  • Excellent communication and organizational skills
  • Nursing: 1 year (Preferred)
  • Acute Care: 1 year (Preferred)
  • BLS Certification (Preferred)
