Requirements

  • Graduate of an accredited nursing program
  • Current, unrestricted Registered Nurse (RN) license in the state of employment
  • Minimum of one (1) year of clinical nursing experience
  • Ability and willingness to travel throughout the assigned service area (approximately 50%)
  • Reliable transportation, valid driver's license, and current auto insurance meeting state requirements
  • Ability to lift, push, pull, and support up to 50 pounds, including assisting with patient transfers and moving equipment

Nice To Haves

  • Associate or bachelor's degree in nursing (ADN or BSN)
  • Previous Home Health or Hospice experience
  • Experience using Homecare Homebase (HCHB)

Responsibilities

  • Complete comprehensive admission assessments to evaluate patients' physical, emotional, psychosocial, and environmental needs
  • Develop and initiate individualized plans of care in collaboration with physicians and the interdisciplinary care team
  • Perform initial nursing evaluations and ongoing reassessments as needed
  • Accurately document patient assessments, clinical findings, and care plans in the electronic medical record
  • Communicate changes in patient condition promptly with physicians, Clinical Team Managers, and other members of the care team
  • Ensure compliance with HIPAA regulations, company policies, and clinical standards
  • Identify safety concerns and report significant findings to appropriate leadership
  • Promote continuity of care while supporting positive patient outcomes
  • Perform additional responsibilities as assigned to support patient care and branch operations
