Pathways Hospice has an immediate opening for an Admissions Coordinator I position. This role provides excellent service to internal and external customers regarding Hospice, Palliative, and Home Health services. The coordinator is responsible for coordinating the intake of referrals and admission of patients according to admission criteria and appropriateness. They will collaborate with team members regarding appropriate programs and assessment visits, answer incoming calls, and provide information about care programs and admission criteria. The position involves explaining care options to patients, families, and facilities, working effectively with discharge planners and referral sources, and ensuring timely patient care by working closely with Business Development and/or Clinical teams. The role includes receiving and documenting referral information, scheduling assessments, determining insurance coverage, and coordinating the admission process. Additionally, the coordinator will collect and verify insurance information, collaborate with the medical community, complete data entry, present admission paperwork, and act as an ambassador by providing tours and general information about services.
