About The Position

Pathways Hospice has an immediate opening for an Admissions Coordinator I position. This role provides excellent service to internal and external customers regarding Hospice, Palliative, and Home Health services. The coordinator is responsible for coordinating the intake of referrals and admission of patients according to admission criteria and appropriateness. They will collaborate with team members regarding appropriate programs and assessment visits, answer incoming calls, and provide information about care programs and admission criteria. The position involves explaining care options to patients, families, and facilities, working effectively with discharge planners and referral sources, and ensuring timely patient care by working closely with Business Development and/or Clinical teams. The role includes receiving and documenting referral information, scheduling assessments, determining insurance coverage, and coordinating the admission process. Additionally, the coordinator will collect and verify insurance information, collaborate with the medical community, complete data entry, present admission paperwork, and act as an ambassador by providing tours and general information about services.

Requirements

Minimum Education: High school diploma.
Minimum Experience: One (1) year clerical/customer service experience (in a healthcare environment preferred).
Ability to lift/carry a minimum of 30 lbs

Nice To Haves

Degree from an accredited university in medical, business or human relations preferred.

Responsibilities

Provides excellent service to internal and external customers in regards to Hospice, Palliative and Home Health services.
Coordinates the intake of referrals and admission of patients according to admission criteria and appropriateness.
Collaborates with team members as to the appropriate programs and requests assessment visits.
Answers incoming calls and provides information regarding hospice, palliative and/or home health care programs and admissions criteria to external and internal customers.
Provides information to patients, families and facilities to explain care options including in the home, long-term care, assisted living care and/or hospice inpatient facility settings.
Effectively works with discharge planners and referral sources.
Works closely with Business Development and/or Clinical team/staff to ensure timely patient care.
Receives and documents complete referral information, schedules assessment using the computer-based appointment system, determines insurance coverage, and helps coordinate admission process.
Collects and verifies insurance coverage information and forwards to appropriate hospice staff as requested.
Collaborates with the Medical Community via phone, EMR system, email, etc. to ensure timely customer service and communication.
Efficiently and accurately completes all data entry.
Initiates and presents admission paperwork to appropriate patients and families based on the need, payer source, financial benefit, and legal assistance.
Acts as an ambassador providing tours and general information about services provided by the hospice palliative and/or health care programs.
