About The Position

The ADHC Social Worker provides intake and patient advocacy services and coordinates social service matters with physicians, staff, participants or patients, families, and other organizations. The role works to reduce barriers to participants’ access to wellness and medical or rehabilitative care across the continuum. Patient-facing work requires proficiency in English and Korean. The Social Worker serves as a resource to licensed and other care providers by helping participants access community resources, arranging medical equipment or supplies needed for a return home, making appropriate referrals to state and government agencies and community self-help organizations, and representing the ADHC at meetings with community health, educational, and welfare agencies.
