Provide best practice assessment, treatment, navigation and support to individuals residing within a Home First transitional residential community program, with a primary diagnosis of substance use disorder. This role involves working collaboratively with an interdisciplinary team to enhance clients' skills and abilities to live independently following discharge from a transitional housing program. The position requires liaising with various internal and external teams, including psychiatric and medical units, the Addictions Medicine Consult Team (AMCT), Rapid Access Addictions Medicine (RAAM) clinic, Outpatient Addictions and Gambling Service (OAGS), and community agencies to ensure continuity of care and support service needs. The Addictions Mental Health Worker will complete assessments, develop and implement treatment plans, document progress, and act as a resource for client and caregiver education. Additionally, the role involves facilitating education and group treatment/counseling sessions using strength-based, therapeutic techniques such as harm reduction and motivational counselling. The position also contributes to learner development, participates in quality improvement initiatives, and represents the department on committees.
