About The Position

This role is within the Supportive Housing Team, focused on assisting individuals who may experience addictions and/or mental health concerns and have faced housing instability. The program provides affordable, independent apartments with on-site supports, enabling early intervention to prevent issues from escalating and to maintain housing stability. The team also fosters a sense of community through groups and low-barrier support services within the buildings, including system navigation, crisis intervention, and supportive listening.

Requirements

  • Completed post-secondary education in a relevant field of study.
  • Current, satisfactory Police Vulnerable Sector Check (PVSC).
  • Demonstrated ability to attend work regularly.
  • G level Driver’s licence.
  • Access to a personal vehicle and minimum $2 million liability insurance to transport clients.

Nice To Haves

  • First Aid & CPR certification.
  • Training in crisis intervention and crisis resolution, including ASIST training program.
  • Previous experience working in the mental health and addictions field.
  • Experience working with individuals with housing instability.
  • Knowledge of Psychosocial Rehabilitation.
  • Knowledge of community resources.
  • Bilingual in both official languages.

Responsibilities

  • Provide support based on the presenting needs of the client.
  • Motivate and support clients to work towards achieving their needs and/or goals.
  • Assist and coach clients in learning and managing activities of daily living such as personal hygiene, chores, budgeting, and socialization within their apartments.
  • Advocate, refer, and connect clients with appropriate services, supports, and resources (formal and informal) based on their needs.
  • Support clients with physical and mental health concerns, and liaise with clinical staff.
  • Coordinate recreational community outings and activities to enhance clients’ quality of life and connect them to their community.
  • Champion each client’s individual capacity for independent community living.
  • Provide health teaching about mental illness and symptom management.
  • Assess clients’ strengths, abilities, needs, and preferences, as well as social and environmental factors impacting the client, and any vulnerability and risk factors.
  • Ensure ongoing assessments are completed and reviewed according to policy.
  • Include and encourage clients’ natural supports (e.g., family, significant friends) in goal planning, clarifying the roles each plays towards each client’s rehabilitation.

Benefits

  • Starting rate of $27.14 per hour (Level 1 – Direct Service), with a wage differential ($30.01 per hour for Level 1 – Direct Service, Regulated) for those registered with designated professional colleges.
  • Grid progression based on seniority up to Level 8 ($34.41 per hour for non-regulated, and $37.26 per hour for regulated).
  • Premiums of $2.00 per hour for eligible Evening/Night (8:00pm – 7:00am) and Weekend Shifts.
  • Immediately eligible for optional enrolment in the Healthcare of Ontario Pension Plan (HOOPP).
  • 9% in lieu of benefits.
