About The Position

The Supportive Housing Worker will assist with the delivery of community support services for individuals in the Clustered Apartments Program. This role involves coordinating mental health services, developing and linking resources within the community, and functioning as part of a multi-disciplinary team to provide comprehensive assessment and treatment services to clients. The Clustered Apartments Program offers on-site monitoring and interventions for individuals requiring an intensive level of care and support, focusing on daily living skills and providing low-income subsidized housing assistance to persons with disabilities, mental illnesses, and/or poverty.

Requirements

HS Diploma/HSE/GED is required.
Must hold or obtain by start date, MO Class E Driver's License (Class D for IL Residents), and proof of continuous auto insurance.
Must meet CCC's Motor Vehicle Record standards policy and have reliable transportation.
Reliable transportation, valid driver's license, and proof of continuous auto insurance are required due to staff transporting clients in extenuating circumstances.

Nice To Haves

Two years of mental health or direct health care experience preferred.

Responsibilities

Provide training to clients in daily living skills.
Facilitate development and/or enhancement of personal support systems within the client's community.
Assist clients in creating and participating in recreational activities as indicated or available.
Perform bimonthly apartment inspections and provide input/skills training on household management.
Maintain work productivity standards as established by supervisor and in accordance with assigned caseloads.
Document according to clinical policies and procedures, DMH, CARF, and any other federal or state program per agency standards.
