About The Position

The Addiction Care Coordinator plays a vital role in helping individuals affected by substance use disorders access compassionate, recovery-oriented care. Depending on the position, Care Coordinators work either in our hospital or outpatient NewStart Clinic, supporting patients at different stages of their recovery journey while using a harm reduction approach that meets each person where they are. In our hospital-based role, Care Coordinators partner with patients during hospitalization to assess needs, provide education, and connect them with appropriate treatment and community resources after discharge. They collaborate with hospital care teams, patients, families, and community providers to help ensure smooth transitions into ongoing care. In our outpatient NewStart Clinic, Care Coordinators support patients throughout their treatment experience by coordinating services, removing barriers to care, and serving as a co-facilitator in our Intensive Outpatient Program (IOP) alongside a licensed therapist. The IOP provides a supportive, therapeutic group environment focused on recovery education, skill building, reducing substance use and harm, and improving functioning across health, home, purpose, and community. No two days are the same. The ideal candidate enjoys variety, is highly organized, works independently, communicates effectively with multidisciplinary teams, and is passionate about helping individuals navigate recovery with empathy, respect, and hope. Care Coordinators work collaboratively with Addiction Medicine physicians, therapists, nurses, and fellow Care Coordinators, while receiving regular clinical supervision and support from the NewStart leadership team.

Requirements

  • Associate degree in human services or related fields
  • 1 year experience working with people with substance use disorder.
  • Experience working in a healthcare setting
  • Experience with Motivational Interviewing
  • Eligible to apply for SAC-IT upon hire
  • Familiarity with the ASAM criteria
  • Ability to manage multiple ongoing care coordination cases efficiently
  • Practice is informed by a harm reduction approach to client interactions
  • Exercise good judgment, maintain good interpersonal relationships, adapt to change, evaluate critically and think creatively.
  • Effective written and verbal skills allowing communication with physicians, co-workers, other clinic/hospital personnel, as well as the public.

Nice To Haves

  • Bachelor degree in human services, psychology, social work, or related field
  • Experiencing using an Electronic Medical Record – ideally EPIC.
  • SAC-IT, SAC, CSAC, CSW
  • Working knowledge of Madison community resources
  • Familiarity with: Community based support groups
  • Familiarity with WI Department of Health Services Chapter 75, “Community Substance Abuse Service Standards”
  • Familiarity with Electronic Health Records
  • Familiarity with Crisis Prevention/Intervention techniques.
