About The Position

The Licensed Practical Nurse serves as a member of the healthcare team at UVA Health Northern VA Culpeper, establishing an authentic, personalized relationship with patients and their chosen support systems and collaborating with team members to promote optimal health. The LPN uses scientific evidence and compassion to assist in patient care throughout the health journey and is accountable for providing care as part of an RN-led team in accordance with current policies and procedures. The role provides and documents patient care under the direction and supervision of the Assistant Nurse Manager or Nurse Manager. This is a PRN, as-needed position.

Requirements

• High school diploma.
• Graduate of an accredited Practical Nursing Program and completion of a nursing program approved by the State Board of Nursing.
• Applicable state LPN license.
• Ability to multitask and prioritize quickly.
• Ability to communicate verbally and in writing.
• Basic computer knowledge.

Nice To Haves

• One year of relevant experience preferred.

Responsibilities

• Provide and document patient care under the direction and supervision of the Assistant Nurse Manager or Nurse Manager.
• Participate in collecting patient data and evaluate patient care needs based on gathered data, health status, and treatment regimen.
• Identify relevant resources based on patient needs and availability.
• Participate in interdisciplinary care planning and collaborate with the registered nurse to modify the care plan as needed.
• Identify nursing interventions and patient goals and review goals with the registered nurse.
• Identify opportunities for process improvement, evidence-based practice, and research.
• Engage in self-reflection and develop professional goals to enhance knowledge, pursue certification, or pursue an academic degree, and integrate gained knowledge into practice.
• Perform additional duties as assigned.
