About The Position

Easterseals PORT Health is seeking an ACTT Registered Nurse to work as part of an Assertive Community Treatment Team (ACTT), alongside professionals including a psychiatrist, licensed clinician, co-occurring disorders specialist, mental health nurse, and other clinical and paraprofessional staff. The nurse assesses, plans, develops, coordinates, and provides treatment, rehabilitation, and support services to help individuals build the skills and supports needed to live, learn, work, and participate in the community with as much independence and self-determination as possible. The position provides nursing services, including medication administration and monitoring, nursing assessments, and interventions. Under the team psychiatrist’s direction, the nurse develops, revises, and maintains medication protocols, policies, and procedures; records and administers prescribed psychotropic medications; and evaluates and documents medication effectiveness, complications, and side effects. Nursing staff perform these duties within their licensure and scope of practice; LPNs work under an ACT team RN’s supervision. The role also provides structured face-to-face therapeutic interventions, health education, care coordination, and support across functional domains. Although there is daily office-based work, most services are delivered in the community, including in streets, homes, shelters, jails, and hospitals, where and when individuals need them.
