About The Position
The nurse/case manager role on the Assertive Community Treatment (ACT) teams is primarily focused on nursing services to adults diagnosed with serious, long-term mental illness. Services shall include needed nursing and medical care as well as case management coordination and activities of daily living assistance to assure a comprehensive and holistic approach to meet individual needs and assist the consumer in obtaining and maintaining psychiatric stability.
Requirements
- RN or Licensed Practical Nurse.
- Minimum 2 years nursing experience.
- Experience working with adults with long-term serious mental illness preferred.
Nice To Haves
- Bachelor's degree in nursing or related health/mental health field.
