About The Position

The Assertive Community Treatment (ACT) Program Care Manager position is responsible for all the functions of ACT Program services, including advocacy, assessment, linking, coordination, and monitoring of services for an assigned caseload of individuals with a primary diagnosis of mental illness.

Requirements

  • Bachelor's Degree in Human Services Field from an accredited school.
  • Must qualify as QMHP, QIDP or CMHP where appropriate for population being served.
  • Valid Driver's License.
  • Chauffeurs license; if transporting individuals that receive services.
  • Knowledge of Word Processing software.
  • Recommended typing speed of 45 wpm.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
  • Ability to deal with problems involving several concrete variables in standardized situations.
  • Regularly required to sit and talk or hear.
  • Occasionally required to stand; travel between offices and throughout building, use hands to finger, handle, or feel objects, tools, or controls; and reach with hands and arms.
  • Must occasionally lift and/or move up to 25 pounds.
  • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

Nice To Haves

  • Licensure as a RN, LLBSW, LBSW, LLMSW, LMSW, TLLP, LLP, LLPC, or LPC.
  • Substance license preferred; staff are encouraged to apply for MCBAP development plan.
  • Experience working with individuals having a mental illness.
  • Professionally licensed, certified, or registered by State or national organization to provide health care services (i.e. LMSW, LBSW LLP, LPC).
  • Must qualify as QMHP or QMRP.
  • Employee may be required to travel to outside meetings or homes as determined by caseload.
  • While performing some of the duties of this job, the employee is will be exposed to outside weather conditions.
  • Having lived experience with behavioral health issues.

Responsibilities

  • Developing a thorough understanding of Michigan mental health code and standards, as well as the standards of funding and certifying agencies such as Department of Mental Health, Medicaid and CARF, regarding ACT Services.
  • Providing on-going screening and assessment of individuals referred whose presenting problems, diagnosis and/or treatment needs suggest a primary concern of mental illness.
  • Securing information such as psychological and social history contributing to the client's situation and evaluating these and the client's needs and capacities to determine least restrictive environments or services.
  • Initiating and coordinating professional assessments according to presenting problem and relevant standards, and sharing and presenting assessment/treatment data to the team and as authorized and required for implementation and coordination with other agencies, courts and caretakers.
  • Assuring, as needed, but at least annually, an evaluation of the need for continued services, the development of an integrated plan of service and engaging in continued compliance with documentation, billing and other requirements to obtain maximum reimbursement.
  • Completing the individual plan of service and quarterly reviews of progress, and integrating the preferences and satisfaction of the individual served in the treatment/training plan and goals.
  • Completing Locus assessments annually, quarterly and post hospital stay or as needed due to significant changes for the individual.
  • Making contacts with assigned individuals and assuring the implementation, coordination and monitoring of the plan of service in residential, employment, community and other settings.
  • Providing consultation and in-service training to caretakers, families and other agencies as needed to meet the specialized needs of individuals served.
  • Referring individuals to community resources as needed and appropriate, and encouraging and supporting them in their efforts to utilize integrated services and resources appropriately.
  • Participating in and assisting with placement planning, completing necessary forms and paperwork for placement.
  • Helping individuals to understand their feelings, attitudes and patterns of behavior.
  • Maintaining ongoing communication with supervisor regarding assigned duties.
  • Participating in conferences and engaging in training and in-service presentations, and in public relations speaking engagements.
  • Performing crisis intervention duties, as assigned.
  • Participating in after hours on call.
  • Performing other duties as assigned.
