About The Position

The mission of the CN Guidance FACT Team is to deliver comprehensive and effective services to individuals who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches. The Forensic ACT Team provides an integrated set of evidenced-based treatment and support services through a highly individualized approach. Services are provided in the community in places and situations where problems arise.

Requirements

  • Bachelor, master's degree or higher, in social work, psychology, rehabilitation counseling, or related field preferred.
  • Peer counselor certification is also accepted.
  • Current New York State Driver's License
  • Clean DMV Record
  • Proof of Motor Vehicle Insurance
  • One year experience working in a mental health setting at the masters or higher level or two years' experience working in a mental health setting at the bachelor's level preferred.

Responsibilities

  • Manages medical records; operates and coordinates the management information system.
  • Ensures that a complete case record is maintained for all recipients in accordance with recognized and acceptable principles of record keeping.
  • Responsible for Quality Improvement, Utilization Review, and Performance Improvement activities including but not limited to data collection, analysis of outcome trends, and identification of service provision, which needs improvement, completion of case records, and the timely submission of required documentation.
  • Plan and coordinate all services, in the office or in the field, which support and supplement the rendering of case management and care coordination services to recipients.
  • Ensure that service plans reflect the recipient's needs. Reviews clinical documents to ensure that revisions to service plans and/or other clinical documentation are done in a timely manner, using appropriate clinical language and properly reflect any changes in recipient's' needs.
  • Must have an understanding of concrete services, i.e., Medicaid, Medicare, DSS and SPOA housing applications.
  • Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress
  • Complete QI Training during onboarding and participate in QI refresher training as needed.

  • Provides direct, face-to-face, Telehealth and/or collateral services to recipients either in the FACT Office or in the community in places and situations where problems arise.
