About The Position

Adult Community Clinical Services (ACCS) is a Department of Mental Health funded service designed to deliver evidence-based interventions within a clinically focused model. Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, and group living environments. Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. The teams are tight knit, supportive, and multidisciplinary. Open Sky’s talented clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff provide quality, compassionate care to the individuals served. Team Leaders provide supervision to the Integrated Team as well as direct supervision and support to Outreach Counselors, Housing Coordinators, and the Administrative Assistant. They also collaborate with individuals served, clinicians, nurses, and other members of the team to implement and monitor any self-management and health and wellness plans and activities.

Requirements

  • High School Diploma, GED or equivalent required.
  • Valid Driver's License and acceptable driving record, required.
  • Minimum of five years experience working in direct care, required.

Responsibilities

  • Hire, supervise, mentor and evaluate staff.
  • Ensure optimum coverage through the development and monitoring of the schedule.
  • Responsible for the implementation of high quality rehabilitation utilizing evidence based practices according to each person’s Treatment Plan, and with supervision from a Licensed Clinician.
  • Facilitate and monitor the implementation of activities that create opportunities for the development of valued roles and personal relationships in the community.
  • Participate in On-call Beeper rotation.
  • Provides supervision to the Integrated Team as well as direct supervision and support to Outreach Counselors, Housing Coordinators, and the Administrative Assistant.
  • Collaborate with individuals served, clinicians, nurses, and other members of the team to implement and monitor any self-management and health and wellness plans and activities.
