About The Position

CareCollab is seeking an Account Manager – Community Partnerships & Referrals to develop, strengthen, and maintain relationships with organizations that can refer eligible individuals to CareCollab’s programs. This is a highly relationship-oriented role for someone who enjoys being out in the community, meeting people, building trust, and creating long-term partnerships. The Account Manager will serve as a key point of contact for CareCollab’s referral partners and will work to ensure that partners understand who we serve, how our programs work, and how to connect individuals to our services. The ideal candidate is a strong communicator and relationship builder who is comfortable working independently, managing multiple relationships, and representing an organization professionally throughout the NYC community.

Requirements

  • 3+ years of experience in account management, business development, community outreach, healthcare partnerships, social services, or a related field.
  • Experience developing and maintaining professional relationships with external organizations.
  • Excellent verbal and written communication skills.
  • Strong interpersonal skills and ability to build trust with individuals at all levels of an organization.
  • Comfortable conducting presentations, meetings, and outreach independently.
  • Highly organized with strong follow-through and attention to detail.
  • Ability to manage multiple relationships and priorities simultaneously.
  • Comfortable working in the field and traveling throughout NYC.
  • Proficiency with CRM systems, Microsoft Office/Google Workspace, and other business technology.
  • Must be comfortable working in a fast-paced, growing organization where processes and priorities may evolve.

Nice To Haves

  • Experience working in healthcare, behavioral health, social services, Medicaid, care management, housing, or a related environment strongly preferred.
  • Strong understanding of the NYC healthcare and/or social-service landscape preferred.

Responsibilities

  • Develop and maintain strong, productive relationships with healthcare providers, hospitals, behavioral health organizations, social service agencies, housing organizations, community-based organizations, and other potential referral sources.
  • Develop a portfolio of community partners and establish regular communication and engagement with each organization.
  • Serve as a trusted point of contact for referral partners and ensure they have a clear understanding of CareCollab’s services and eligibility criteria.
  • Conduct regular in-person visits, meetings, presentations, and outreach activities throughout NYC.
  • Identify opportunities to deepen existing relationships and expand CareCollab’s presence within partner organizations.
  • Represent CareCollab at community events, networking opportunities, conferences, and other relevant gatherings.
  • Identify and cultivate new sources of qualified referrals.
  • Educate partners on CareCollab’s programs, target populations, referral process, and value proposition.
  • Proactively follow up with referral sources to understand barriers, answer questions, and encourage continued referrals.
  • Monitor referral activity across assigned partners and identify opportunities for increased engagement.
  • Work closely with internal teams to ensure referral partners receive a responsive and positive experience.
  • Communicate partner feedback and referral trends to the Business Development and leadership teams.
  • Develop strategic account plans for key referral partners.
  • Maintain consistent touchpoints with partners based on the needs and potential of each relationship.
  • Track meetings, outreach, referrals, and other partnership activity in the company’s CRM.
  • Monitor the health and engagement of partner relationships and proactively address concerns or challenges.
  • Identify opportunities for cross-organizational collaboration, education, and programming.
  • Ensure commitments made to partners are followed through and that partners receive timely communication.
  • Develop a strong understanding of NYC’s healthcare and social-service landscape.
  • Identify emerging organizations, programs, and referral networks that may be valuable to CareCollab.
  • Stay informed about changes in Medicaid, Health Home, behavioral health, housing, and social-service systems that may impact referral opportunities.
  • Attend community meetings and networking events to expand CareCollab’s visibility and referral network.
  • Build relationships across multiple levels of partner organizations, from frontline staff to organizational leadership.
