About The Position

The Access Center Intake Coordinator position will support the flow and responsibilities of Access Center Programming and Case Management. The Access Center Intake Coordinator will have a caseload of members working towards a variety of goals. This role is an integral part of case management services at SPY.

Requirements

  • 1-2 years of experience in social services or related field is required.
  • 1+ years of experience working in an Access Center environment.
  • 1+ years of experience providing direct services to high-risk youth.
  • Working knowledge of child and adolescent social, emotional, and cognitive development, trauma informed care and harm reduction service model
  • A commitment to and passion for the mission, vision, values and culture of SPY
  • A self-starter who is prepared to drive innovative solutions for an emerging organization
  • An ability to work in a fast paced environment that requires effective collaboration with a variety of organizations and individuals
  • Must possess great communication and organizational skills
  • Proficient in Microsoft Office and Google Drive
  • Must have reliable transportation, valid California driver's license and proof of current insurance.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Changes in the environment, such as the office or outdoors.
  • Physical effort/lifting, such as sedentary- up to 15-20 pounds at times.

Nice To Haves

  • Bachelor's Degree in Social Work, Psychology, Sociology or related field is preferred.
  • 1+ years experience providing direct services to high-risk youth including counseling and advocacy is preferred.

Responsibilities

  • Support Access Center Case Management Team in day to day operations and programing of the Access Center
  • Run Front Desk operations on a rotation basis ensuring members are connected with needed programs and services
  • Conduct intakes, assess needs, and refer new youth to needed services/programs such as housing, mental health, benefits, education, and employment as needed
  • Use a harm reduction, trauma informed, housing first model when working with youth
  • Provide mediation and crisis intervention to youth as needed
  • Participate in team approach to developing youth driven treatment plans, case summaries, and progress reports as needed
  • Work within the Coordinated Entry System (CES), utilizing HMIS and SPY Database to track and record Drop-in Case Management data
  • Prepare data collection and analysis, monthly reports, and program administration duties in a timely fashion
  • Advocate by attending external community meetings with government agencies and community groups
  • Support members with housing and health and wellness goals
  • Work collaboratively across internal departments and assist with general SPY programming as needed
  • Work within established legal and ethical guidelines
  • Ability to work nights/weekends in near future.
  • Other duties as assigned to support the mission of the program and organization.
