About The Position

Requirements

• Associate’s degree of nursing with RN licensure from an accredited college
• One (1) year working as a registered nurse.
• Current State of Minnesota Nursing Board RN license

Nice To Haves

• Bachelors or Master’s degree of Nursing
• Three years’ experience.
• Experience performing case management in Home Care, hospital, Post-Acute Care, discharge planning, care and/or quality improvement.
• Certified Case Manager through Commission for Case Manager Certification (CCMC) or eligibility to sit for CCM exam within 2 years of hire.

Responsibilities

• Provides inpatient vs Hospital care management services for medical/surgical patients to facilitate optimal patient outcomes.
• Supports the delivery of effective and efficient care.
• Improves continuity of care and efficient use of resources to progress the plan of care.
• Develops a discharge plan to prevent readmissions and patient focused.
• Collaborates with the care team including, but not limited to: attending/specialty physician(s), Social Work, Utilization Review, therapies, and Nursing to ensure that an effective and timely treatment plan is acted upon, and appropriate discharge plan is implemented with a smooth transition in levels of care.
• Provides Care Management services on weekends and/or holiday(s) rotations as determined by leadership.
