About The Position
Provides direct and indirect nursing care to assigned patients, working under the direct and indirect supervision of the Nurse Manager or designee while independently applying nursing knowledge and skills and managing clinical situations. The role coordinates care across the continuum for Heart Failure patients in collaboration with Heart Failure cardiologists, advanced practice nurses, and other members of the multidisciplinary team. It supports patients and families in accessing community resources and transitioning between care settings, provides education on managing Heart Failure at home, oversees clinic data quality and performance improvement, collaborates with the inpatient Heart Failure team, and coordinates telemedicine data between clinic providers and patients. The position is based at the Advanced Heart Health Center at Prisma Health Heart Hospital, a multidisciplinary center caring for patients with complex heart disease. This is a non-management position reporting to a supervisor, manager, director, or executive. The position is eligible for a $5,000 sign-on bonus for external candidates only.
Requirements
- Associate degree in Nursing.
- At least one year of nursing experience.
- Current RN compact/multistate license recognized by the NCSBN Compact State, or a license to practice as an RN in the state where the team member works.

- Bachelor's degree in Nursing preferred.
- Cardiac nursing experience preferred.
Responsibilities
- Collect patient data, complete required forms, identify patient problems and needs, set priorities, identify issues requiring referral or follow-up, interpret and record diagnostic results, and perform nursing assessments using critical thinking.
- Develop and update plans of care using the nursing process, incorporate plans from other disciplines and continuing care needs, involve patients and families, and make referrals to multidisciplinary support services.
- Provide prescribed treatments and procedures in accordance with accepted practice and patient care standards; explain procedures, provide discharge teaching, recognize emergencies and initiate advanced therapy, advocate for patients, coordinate care, and evaluate care measures.
- Record care delivered and variations with rationale, document observations of impending or associated problems, and implement appropriate nursing measures.
- Delegate nursing care appropriately, integrate cost-effective practices, participate in quality improvement, document unit problems, meet staffing demands based on patient care needs, monitor safe working conditions, address ineffective care management, complete written reports, fulfill assigned unit call, and adjust staffing for census and acuity.
- Coordinate Heart Failure patient care across the continuum with cardiologists, advanced practice nurses, and the multidisciplinary team.
- Build therapeutic relationships with patients and families and facilitate access to community resources to support transitions.
- Teach and develop curriculum to reinforce education for Heart Failure patients and their families on home self-care.
- Oversee data quality and performance improvement in the clinic.
- Collaborate with the inpatient Heart Failure team to support care transitions.
- Monitor and coordinate telemedicine data between clinic providers and patients.
- Perform other duties as assigned.
