About The Position

The Case Manager is responsible for coordinating services and providing direct support to youth residing in Centerboard's Youth & Young Adult Group Residence (YYAGR) program. This position plays a key role in client intake, treatment planning, case management, discharge planning, and collaboration with schools, DCF, healthcare providers, and other community partners. The Case Manager works closely with residents, families, collateral contacts, and program staff to support youth in achieving treatment goals, developing independent living skills, and preparing for successful transitions into adulthood. The Case Manager also provides leadership and guidance to direct care staff, ensures documentation meets program and regulatory requirements, and assists with overall program operations.

Requirements

  • Bachelor's degree in Psychology, Social Work, Human Services, or a related field required.
  • Minimum of two years of experience working with youth in a residential, behavioral health, or human services setting preferred.
  • Strong communication, documentation, organizational, and case management skills.
  • Ability to work effectively with youth experiencing behavioral, emotional, and social challenges.
  • Ability to collaborate with families, schools, DCF, healthcare providers, and community partners.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Valid driver's license and acceptable driving record required.
  • Ability to use a personal vehicle when a Centerboard vehicle is unavailable.

Responsibilities

  • Coordinate client admissions, interviews, intakes, and discharge planning.
  • Complete required intake paperwork, assessments, treatment plans, safety plans, and ongoing case management documentation within established timelines.
  • Maintain client records in accordance with DCF, EEC, Caring Together, and Centerboard requirements.
  • Coordinate and facilitate treatment team meetings and communicate regularly with DCF workers, schools, healthcare providers, and other collateral contacts.
  • Monitor client progress toward treatment goals and provide regular updates to treatment teams.
  • Complete and maintain required assessments, including treatment plans, safety plans, risk assessments, YRATs, CANS assessments, biopsychosocial assessments, and discharge documentation.
  • Coordinate resident appointments, transportation needs, and daily schedules.
  • Support residents in developing independent living skills, problem-solving skills, and successful transition plans.
  • Review and oversee staff documentation to ensure accuracy, timeliness, and compliance with program standards.
  • Provide leadership, coaching, and support to direct care staff.
  • Participate in the on-call rotation and document on-call activity as required.
  • Conduct interviews for prospective residents, employees, and interns as assigned.
  • Facilitate resident groups, house meetings, and skill-building activities.
  • Monitor resident budgets and assist with financial planning and independent living goals.
  • Assist with program quality improvement initiatives, including CQI and Theory of Change activities.
  • Transport youth to appointments and meetings as needed.
  • Perform other duties as assigned.
