Reporting to the Youth Connections Team Leader, the Youth Transition Coordinator is responsible for providing holistic, relationship-based prevention support to youth and young adults in Member Communities as they prepare for adulthood and greater independence. As an integral member of a multidisciplinary team, the Youth Transition Coordinator collaborates with young people to develop individualized Transition Plans, build practical life skills, strengthen self-advocacy, connect to culture and community, and address goals related to education, employment, housing readiness, health and wellness, financial literacy, and independent living. The role combines one-to-one support with workshops, groups, events, cultural learning opportunities, and community-based programming. As a member of the SCFSS organization, the Youth Transition Coordinator is committed to the collective response to the Call to Action through the four Strategic Directions and supports a pathway of change to exercise nłeʔkepmx and syilx inherent rights. In particular, the Youth Transition Coordinator champions core elements of the strategic goal of ‘Protection to Prevention’.
