About The Position

The Youth Services Support Program Coordinator helps run the Youth & Family TREE Program, a federally funded initiative supporting adolescents, transitional-aged youth, and families impacted by substance use disorders (SUD) and co-occurring mental health conditions. This role involves day-to-day program management, including scheduling, coordination, communication, event support, partner follow-up, and accurate data tracking and reporting. The coordinator will work closely with the Program Director, internal care teams, and community partners to ensure smooth transitions and strong engagement.

Requirements

  • 3–5 years of related experience
  • Strong organizational skills and attention to detail (especially with data accuracy)
  • Strong communication skills and customer service mindset
  • Ability to collaborate with diverse teams and community partners
  • Proficiency with common office technology (Outlook, Word, Excel, internet tools); ability to learn an EMR/EHR
  • Valid Montana driver’s license
  • Ability to pass a criminal & driving background check

Nice To Haves

  • Associate’s or Bachelor’s degree in Social Work (or related field)
  • Experience in a healthcare setting and/or FQHC
  • Certifications or experience such as Behavioral Health Aide, Mental Health Aide, Medical Assistant, Chronic Care professional, Certified Application Counselor, and/or SHIP

Responsibilities

  • Coordinate daily program operations: scheduling, documentation support, and team communications
  • Serve as a liaison between participants and internal/external service providers to support access and warm handoffs
  • Collect, enter, and maintain accurate participant/program data to meet grant reporting and compliance requirements
  • Support referrals to other Alluvion Health services when appropriate (medical, dental, outreach/enrollment, prescription assistance)
  • Help plan, organize, and attend youth-focused events, coalition meetings, outreach/tabling, and community engagement activities
  • Assist with outreach materials, presentations, informational handouts, and public messaging (including social media support as assigned)
  • Support program quality improvement, audits, and grant-required fidelity standards
  • Maintain HIPAA compliance and confidentiality in all interactions
  • Perform other duties and special projects as assigned
