The Youth and Family Development Associate (YFDA) will provide Transitional Housing Program (THP) Case Management and supportive services to enrolled youth. The YFDA will be responsible for providing access to eligible youth transitional housing with individualized supportive services that assist each youth address the factors contributing to their homelessness. THP is committed to providing services that are trauma informed, strength based, person center and empowerment oriented.

Requirements

  • Bachelor's degree in social work, psychology, or related field.
  • Ability to work with a diverse population, possess cultural sensitivity, and maintain appropriate boundaries with clients.
  • Demonstrated leadership and problem-solving abilities, training skills, creativity, and public speaking.
  • Ability to provide quality trauma-informed services in a fast-paced environment, specializing in crisis intervention and de-escalation, motivational interviewing, and Seeking Safety.
  • Ability to (re) prioritize and multi-task is a must.
  • Available to work one evening per week until 8 p.m.
  • Employment may be conditioned upon satisfactory completion of certain background clearances which may include a Department of Justice criminal background check requiring my fingerprints (Live Scan), Community Care Licensing, screening through the National Sex Offender Public Website, physical exam, drug test, tuberculosis (TB) test, and such additional clearances as may be required under the program.
  • Candidates/employees who have received a conditional offer of employment will be required to undergo testing for controlled substances, (including THC and CBD), in accordance with the County of San Diego's drug and alcohol policy and the federal Controlled Substances Act.
  • Candidate may be required to have reliable transportation; therefore, the candidate must successfully complete a motor vehicle history check. Candidate must also possess and maintain a current valid California driver’s license and valid auto insurance policy.

Responsibilities

  • Support in the implementation and evaluation of THP that provides current/former foster youth quality trauma informed services that adhere to and uphold the standards of SBCS.
  • YFDA role will include the provision of case management, and supportive services to enrolled youth.
  • Conduct a comprehensive assessment that assists youth to identify strengths and supports necessary to obtain and sustain stable, permeant housing.
  • Make referrals to career pathway, life skills, mental health, mentorship, educational and other support programs.
  • Connect youth to support that can assist them to achieve increased economic stability and self-sufficiency.
  • Assist youth to create a budget, identify affordable housing and develop a plan to transition out of transitional housing.
  • Meet with youth weekly to maximize success and promote stability using a step-down strategy for gradually reducing dependence on transitional housing assistance.
  • Must be flexible to provide services in-home, drop-in center, and in the community as needed.
  • Attending all required agency training and meetings.
  • Maintain documentation and reporting on all youth/families in a timely manner.
  • Maintain confidentiality and privacy standards of service partner protected health information and other applicable information and material in accordance with agency and center policies, and applicable regulations and laws, including HIPAA.
  • Maintain warm, welcoming office grounds and facilities.
