Within the context of a client and family centered care model and in accordance with established standards of professional practice, and vision and values of the organization, provides services to children, youth and families including comprehensive psychosocial assessments and individual, group and family counselling. Develops, implements, and adjusts individualized treatment plans for youth with substance misuse issues and who often have mental health issues; establishes trust with youth and engages them at their level of readiness for change. Provides screening and referral services including psychoeducation, family sessions and parent groups; determines appropriateness of request, client eligibility and availability of resources. Collaborates and consults with colleagues, school personnel consultative services and others, attends and participates in meetings, develops linkages with other facilities, agencies, and support groups, and participates in quality improvement initiatives.
