About The Position

Family & Children’s Services is looking for a Youth Care Coordinator to provide targeted support services and Youth-centered Systems of Care (SOC). SOC coordinates mental health and community services with the needs of youth and families impacted by trauma by preventing out-of-home placements and encircling families with support and resources. Family & Children’s Services is Oklahoma’s leading provider of behavioral health care and family services and our Care Coordinators work with children in their homes/communities while incorporating the entire family in treatment options. SOC is designed to focus on the youth's strengths, age and cultural heritage and wraparound services to strengthen healthy home, school and family experiences.

Requirements

  • Bachelors Degree required
  • Experience in the field of mental health, criminal justice, or community based social services required.
  • ODMHSAS Case Manager II Certification, completed within 60 days of eligibility.
  • Wellness Coach Certification Required
  • Must possess a valid Driver License and satisfactory driving record to use agency and/or personal automobile to travel to locations other than primary office and/or for the transportation of clients.

  • Bachelor's Degree in Social Science preferred.

Responsibilities

  • Conduct comprehensive assessments of clients' physical, psychological, social, and environmental needs.
  • Implement individualized care plans in collaboration with clients, families, and other stakeholders that address the unique needs and goals of clients, promoting their reintegration into the community through tailed support and skill-building activities.
  • Coordinate and monitor the delivery of services to ensure timely and appropriate care.
  • Advocate for clients' rights and assist them in accessing necessary resources and services.
  • Provide emotional and practical support to clients and their families.
  • Regularly evaluate clients' progress and adjust care plans as needed.
  • Maintain accurate and up-to-date documentation of clients' care, progress, and outcomes.
  • Facilitate communication and collaboration among multidisciplinary teams, including healthcare providers, social workers, and therapists.
  • Participate in case conferences, team meetings, and other collaborative activities to ensure coordinated care.
  • Educate clients and families about available services and resources and assist them in making informed decisions.
  • Ensure compliance with all relevant laws, regulations, and standards of practice.
  • Participate in continuing education, training, and professional development opportunities to stay current with best practices and trends in case management.
  • Completing certification renewals and all assigned trainings by deadline.
  • Facilitating communication amongst treatment team, internal and external providers, clients, and families.
  • Coordinating appointments, referrals, and services within the healthcare system.
  • Fulfilling transportation needs as is applicable to the individual program.
  • Ensuring timely and appropriate care transitions.
  • Providing support and education to clients and families regarding upcoming appointments.
  • Obtaining ROIs, making medical records requests, and ensuring the treatment team receives the most up-to-date records.
  • Assisting nursing staff with medical record reconciliation as needed.
