About The Position

Access Services is hiring a Young Adult Mental Health Case Manager who will join our Transition to Independence Process (TIP) program serving across Bucks County, with an office based in Warminster, PA. The Transition to Independence Process (TIP) Program uses an evidence-based model to support transition-age young adults ages 16-26 years old to achieve their vision of a successful future. A Young Adult Mental Health Case Manager is also known at Access by the name, "TIP Facilitator". TIP Facilitators support young adults as they develop life skills, connect to resources, and build overall independence in reaching their chosen goals. TIP isn’t traditional case management – it’s a strength-based, person-centered, and youth-driven approach to mental health support. Your role is to empower young adult in setting and achieving goals, provide in the moment coaching, and facilitate problem-solving processes.

Requirements

  • Bachelor’s degree in social work, psychology, criminal justice, or a related human services field (required).
  • Willingness and ability to travel and transporting youth throughout the community (required)
  • Strong communication, organization, and creative problem-solving skills.
  • Ability to work both independently and as a part of a collaborative, strengths-focused team.
  • Knowledge of various mental health diagnoses & disabilities.
  • High emotional intelligence and self-awareness of your impact on others
  • Ability to utilize engagement strategies to support youth and young adults with diverse mental health diagnoses, developmental needs, and varying abilities, including individuals with Level 1 and 2 Autism Spectrum Disorder.
  • Ability to create and implement group event ideas for youth ranging from simple to complex and implement them. recreational, skill building, and community or peer connection
  • Maintain regular and predictable attendance, and work scheduled overtime, or be available for unplanned overtime as necessary, to meet program needs.
  • This job requires full-time, on-site presence due to the nature of the role and responsibilities, with no remote or hybrid work schedule options.
  • This position requires the ability to travel during a normal workday to successfully carry out responsibilities. Therefore, employees will need to drive their personal vehicle and/or an agency owned vehicle to meet these expectations.
  • Employees are expected to always drive safely, to meet eligibility criteria in Motor Vehicle Policy, and to successfully pass the agency’s substance abuse screening at any time during employment.
  • A valid US driver’s license for at least two years, not including time with permit.
  • No serious traffic convictions within the past three years. (Serious convictions include, but are not limited to, DUI, reckless driving, leaving the scene of an accident, or three or more traffic violations and/or at-fault accidents within that period).
  • Access to a reliable vehicle during work hours. Vehicle must have valid registration and inspection stickers.
  • Valid auto insurance. You will be asked to provide proof that you are a covered driver on the policy.

  • Work or internship experience within mental health and community based human services is preferred.

Responsibilities

  • Deliver community-based services daily by driving to and meeting with young adults in their homes and community locations across Bucks County.
  • Engage youth using the TIP Model to build overall independence across five key domains: (Education; Employment & Career; Living Situation; Community Life Functioning; Personal Effectiveness & Wellbeing).
  • Build authentic, trusting relationships with Support young people in creating and following their Futures Plan – which is a personalized roadmap of goals.
  • Attend and advocate alongside of young adults in multi-system meetings and collaborate with other key players including families, schools, employers, and community partners.
  • Plan and participate in group events that promote connection, life skills, and fun!
  • Complete documentation and meet billable hour requirements in alignment with program and regulatory standards.
  • Participate in case management reviews, on-call rotation, and weekly team meetings.
  • Represent the TIP Program through outreach and community engagement efforts.
