The Youth Homeless Demonstration Program (YHDP) Case Manager provides intensive, individualized case management to youth and young adults (ages 18–24) with dependents who are experiencing or at risk of homelessness. The Case Manager partners with participants to develop individualized housing stability plans that promote long-term self-sufficiency through stable housing, education, employment, life skills, and community connections. Using a trauma-informed, strengths-based, and youth-centered approach, the Case Manager assists participants in overcoming barriers while supporting their journey toward independence. This position works collaboratively with community partners to ensure youth receive comprehensive, coordinated services that foster long-term success while maintaining compliance with HUD, Continuum of Care (CoC), and Youth Homeless Demonstration Program (YHDP) requirements.
