About The Position

The Community Service Board of Middle Georgia's IC3 department is looking for our next Wrap Supervisor for the Intensive Customized Care Coordination (IC3) Program! Our Wrap Supervisors manage our Care Coordinators who serve youth and families in need of customized, coordinated care to discover the underlying needs of the family by building upon the strengths of the family and their support system. The Wrap Supervisor will also be responsible for engaging Care Coordinators with assigned families and their formal and informal support to determine all needs and strengths. The Wrap Supervisor will have to be on-top of acute deadlines, ensuring accurate completion of action plans, timely and thorough documentation all while meeting the requirements of our program.

Requirements

  • Master’s Degree in related field (Social Work. Psychology, etc.)
  • Minimum of two (2) years clinical intervention experience in serving youth with SED or emerging adults with mental illness.
  • Effective verbal and written communication skills.
  • Strong interpersonal skills and the ability to work effectively with a wide range of individuals throughout diverse communities.
  • Ability to motivate and develop a team of Care Coordinators.
  • Ability to analyze complex information, and to define and solve problems.
  • Ability to take initiative when needed and be self-motivated.
  • Ability to effectively tell and guide others to tell a family’s story by understanding and relaying their hope for the future.
  • Familiarity with Microsoft 365 Office Products

Nice To Haves

  • Three plus (3+) years clinical intervention experience in serving youth with SED or emerging adults with mental illness.
  • Previous experience with direct care coordination
  • Previous training experience

Responsibilities

  • Provides troubleshooting assistance to Care Coordinators under supervision.
  • Providing/Hosting various levels of supervision throughout each month.
  • Ensures Care Coordinators meet the required time commitment for each of their assigned families and their support.
  • Guiding Care Coordinators to create both formal and informal inclusive support teams for their assigned families.
  • Promote supervision to encourage Care Coordinators to formulate innovative strategies to meet and exceed the unmet needs of families served.
  • Communicating with team on program updates, ensuring follow-ups on strategies and updates regularly.
  • Ensuring and promoting collaboration between Care Coordinators and assigned Certified Peer Specialists to understand their knowledge of families served as well as their ability to support those served as needed.
  • Regularly attend training courses and conferences to remain up to date of industry knowledge ensuring the growth and development of Care Coordinators.
