About The Position

As a Wraparound Facilitator, you can meet youth and families where they are at and play an important role in getting them where they want to be in a collaborative environment where Wraparound Facilitators have the tools, time, and training to provide excellent services in a positive team environment. Wraparound Facilitators work in homes, schools, and with other system partners to help build a network of care around each family that we serve. Wraparound is an intensive care coordination service that serves youth with complex emotional and behavioral needs who are at risk of out-of-home placement and are involved with multiple human service systems. Wraparound Facilitators lead the effort to promote youth and family voice, choice, and strengths to empower and support Youth and Family Teams to meet their goals.

Requirements

  • Valid MI driver's license required.
  • Some after-school & evening hours required to meet the needs of our youth and families.
  • Candidates must be committed to our shared values and commitment to community care including clinical excellence, community inclusion, community benefit, innovation, integrated care and improvement.
  • Strong teaming skills required.
  • Case Manager Level I: Bachelor's degree in a human services related field OR bachelor's degree in social work. If degree in social work, licensure as LBSW or LLBSW required. If licensed at LLBSW level, individual must obtain full LBSW within two years of hire.
  • Case Manager Level II: Master's degree in social work or equivalent with LLMSW or equivalent required. If licensed at LLMSW level, individual must obtain full LMSW within two years of hire.
  • Case Manager Level III: Master's degree in social work or equivalent with LMSW, LPC or LLP.

Responsibilities

  • Collaborate with youth, caregivers, system partners, and natural supports to plan and lead Child and Family Team meetings.
  • Identify strengths, needs, and develop a family-driven, youth-guided, team-based Wraparound Plan to implement services and supports.
  • Lead meetings in homes and in the community with youth, families, natural supports, and system partners including schools, Juvenile Court, mental health providers, MDHHS, and more.
  • Connect families with resources and supports.
  • Create individualized Crisis and Safety Plans.
  • Work with the Child and Family Wraparound team to prevent out-of-home placement by coordinating services and helping youth and families remain connected to their supports in the community.
  • Write agendas and plan Child and Family Wraparound meetings.
  • Participate with a treatment team to provide crisis intervention to reduce the risk of hospitalization or out-of-home-placement for a youth.
  • Partner with families by providing advocacy and support at IEP meetings, Juvenile Court hearings, assessments, and other service-related appointments.
