Position Summary The Wraparound Facilitator provides direct service coordination and implementation of Wraparound services for children and families participating in Safe at Home (SAH), Children with Serious Emotional Disorders (CSED), and other Wraparound programs as assigned. This role serves as the primary facilitator of the treatment team process, helping families identify strengths, develop natural supports, access resources, and achieve goals that promote long-term stability and success.

Requirements

  • Bachelor's Degree from an accredited college or university in Social Work, Human Services, Psychology, Counseling, Sociology, or a related social services field.
  • Minimum of two years of experience working with children and families.
  • Knowledge of child welfare, behavioral health, Wraparound services, and trauma-informed care preferred.
  • A Master's Degree in a related social services field may substitute for required experience.
  • Passionate about serving vulnerable children and families.
  • Thrive in community-based and family-centered environments.
  • Possess strong communication, organization, and relationship-building skills.
  • Comfortable working independently while collaborating with multidisciplinary teams.
  • Understand trauma-informed and strengths-based approaches to service delivery.
  • Can effectively balance documentation requirements with direct client engagement.
  • Maintain a valid driver's license, reliable transportation, and auto insurance.

Nice To Haves

  • Licensure through the West Virginia Board of Social Work Examiners or an equivalent licensing board is preferred but not required.

Responsibilities

  • Facilitate family-centered Wraparound services using evidence-based and trauma-informed practices.
  • Coordinate and lead treatment team meetings while monitoring client and family progress.
  • Partner with families to identify and engage formal and natural supports.
  • Collaborate with schools, community agencies, healthcare providers, and other stakeholders to ensure coordinated service delivery.
  • Guide families through all phases of the Wraparound process, including engagement, planning, implementation, maintenance, transition, and aftercare.
  • Conduct CAPS assessments and maintain required CANS and CAPS certifications.
  • Provide services including parenting support, visitation, transportation, and family engagement activities.
  • Complete comprehensive family assessments and documentation in accordance with program requirements.
  • Conduct weekly face-to-face home visits and maintain ongoing contact based on treatment plan needs.
  • Facilitate monthly Family Team Meetings and participate in MDT and IEP meetings.
  • Support youth transitioning from residential treatment and out-of-home placements back into family and community settings.
  • Educate families on trauma, child development, parenting strategies, behavioral health, and community resources.
  • Maintain compliance with Medicaid, Joint Commission, state, and agency documentation standards.
  • Utilize de-escalation techniques and crisis intervention strategies when appropriate.
  • Participate in a 24-hour on-call rotation.
  • Build and maintain strong working relationships with the Department of Health and Human Resources (DHHR) and community partners.
  • Ensure timely completion of reports, case documentation, assessments, and service plans.
  • Participate in supervision, trainings, meetings, court hearings, and professional development activities.
  • Promote culturally responsive and family-focused service delivery.
  • Maintain a valid driver's license, reliable transportation, and auto insurance.
