About The Position

EveryMind Mental Health Services is currently recruiting for a skilled, experienced, and enthusiastic mental health professional for the position of Wraparound Facilitator to work with various programs and services to provide a continuum of mental health interventions to children/youth and their families on a full-time permanent role. EveryMind is an accredited child and youth mental health organization that provides a continuum of high-quality mental health services for children, youth and their families who are experiencing or are at risk of experiencing mental health, social, emotional and/or behavioural challenges. Our services are provided in a dynamic, client-centred environment that values respect and inclusion, collaboration, learning, innovation, and accountability. The Peel Wraparound Process is a team-based planning approach that is intended to provide families who experience emotional, social, behavioural, or mental health challenges with individualized, coordinated, family-centered care to meet the complex mental health needs of children and youth who are involved with other child/youth/family-serving systems (e.g., child welfare, youth justice, education, and healthcare). Wraparound’s strength-based planning approach helps families by developing a team of both formal and informal supports to “wrap around” them. Wraparound facilitators provide services to children, youth and their families in-home, in-school and in the community. Together with their team (family, friends, chosen providers), families work to identify their priority needs and to develop creative and practical strategies to meet these needs – strategies that are directly linked to the family, their team and the strengths and capabilities of the community. This is a full-time position due to an existing vacancy.

Requirements

  • Have a minimum of three years’ experience working with children/youth and families in a mental health setting and/or developmental services sector.
  • Demonstrated knowledge and commitment to the Wraparound Community Values; Embraces diversity and demonstrates cultural awareness with critical self-awareness and understanding of engagement with diverse cultures and ability to work within a child/youth/family centered, family-systems, strength-based approach.
  • Current knowledge of the legislation for children’s and youth services.
  • Demonstrated family and youth engagement skills, facilitation skills and the ability to build relationships of trust and possess exceptional interpersonal and conflict resolution skills.
  • Demonstrated thorough understanding of systems theory, experience working with its principles and engaging systems to meet goals of children, youth and families.
  • Demonstrated ability to work both independently and collaboratively as a member of a team.
  • Utilize high levels of oral and written communication skills, time management, and critical thinking skills with a strong understanding of role boundaries.
  • Knowledge of client information systems, report writing and Microsoft Office Suite.
  • Have a valid Ontario Driver’s license (Full G Class) and use of a vehicle with a comfort in driving clients throughout the Peel region (surrounding areas).
  • Available to work flexible hours, including evenings, according to client needs.
  • A Satisfactory Vulnerable Sector Police Check and Pre-Employment Medical screening (including TB test, MMR, TD, Polio, Covid-19 Vaccination), is required.

Nice To Haves

  • Bachelor’s degree or 3 Year College Diploma in Social Work or Social Services or a related field with eligibility for membership in the appropriate professional College or Association is an asset.
  • Previous training in Wraparound Process and Model is a decided asset.
  • Proficiency in languages in addition to English is an asset.
  • Group facilitation experience is an asset.

Responsibilities

  • Guided by the community values of the Wraparound Process, you will assist families in a highly diverse ethno-cultural community in developing a Wraparound Service Plan.
  • Employ a variety of systemic and evidence-based/evidenced-informed treatment approaches (i.e. collaborative problem solving, solution focused, motivational interviewing, wraparound processes) utilizing specific strategies to enhance parent/family and child/youth interactions and functioning at home, and in community.
  • You will develop, implement and evaluate wraparound plans according to individual and family/caregiver goals, strengths and cultural values.
  • You will assist families in advocacy, providing linkages with community resources, coordinating access to services and/or treatment and monitoring the alignment of the plan’s implementation to youth/family goals.
  • You will develop strong working relationships with the children, youth and/or their families and be responsible for planning and community mobilization efforts, which support and sustain a community-based, family-driven, strength-based and culturally and linguistically competent system of care.
  • Effectively assess, manage, design, and implement a crisis plan to ensure safety within the home/school/community setting to address the issues and complexities of the child/youth and family.
  • Provide transitional support and discharge planning to youth/caregivers exiting service to sustain and transfer skills.
  • Provide in-person, virtual and telephone sessions as needed.
  • Provide group services as required.
  • Maintain comprehensive case records and data in accordance with the organizations’ standards and timelines.
