Conduct comprehensive patient assessments to determine physical, mental, social, and spiritual needs related to wound, ostomy, and continence care. Develop and document individualized care plans based on patient assessments, selecting appropriate interventions and supplies. Maintain accurate and timely patient documentation in the electronic medical record. Administer treatments and medications safely while monitoring patient response and adjusting care plans as appropriate. Provide direct wound, ostomy, and continence care that promotes healing, patient comfort, and optimal outcomes. Educate patients and families on wound care, ostomy management, continence strategies, self-care techniques, and lifestyle modifications. Provide education and clinical guidance to nursing staff and other healthcare professionals on wound, ostomy, and continence care. Collaborate with physicians, nurses, and interdisciplinary team members to coordinate comprehensive, patient-centered care. Monitor, evaluate, and document patient progress, revising care plans based on patient response and clinical outcomes. Respond to urgent patient care needs and participate in on-call coverage, including weekends and holidays, as required. Assess the educational needs of nursing staff and support competency in skin integrity, wound prevention, and specialty care. Incorporate evidence-based practice, current research, and clinical guidelines into patient care and quality improvement initiatives. Communicate patient care concerns, barriers, and recommendations to the Clinical Nurse Manager and interdisciplinary team to support safe, effective, and coordinated care.
